Exciting News – Our new location is open!
Dear Valued Patients,
We’re thrilled to announce that our practice is growing! To better serve our community and help fill the gaps in rheumatology care, our office has opened a second care center.
Dr. Roman Czubatyj, Dr. Dana Rockwell, Kari Ewalt PA-C are now seeing patients at 3150 Livernois Rd, Suite 275, Troy, MI 48083.
Dr. John Tower, Dr. Mahpara Syed, Tony Brady, PA-C, Fadya Siso PA-C will be continuing to care for patients at our main location within the Wellpointe Building, 1701 E South Blvd, Ste 110, Rochester Hills, MI 48307.
Our new office will offer the same compassionate, high-quality care you’ve come to know and trust.
We understand that the new distance or transportation limitations may pose challenges for some patients. Our dedicated scheduling team is here to work with you to explore options that ensure continuity of your rheumatologic care.

** Scheduling a Visit **
Welcome to Our Clinic!
We are committed to providing timely, high-quality care through a collaborative, team-based approach. Due to the nationwide physician shortage, we have implemented a scheduling model that allows our patients to be seen more quickly and efficiently. All new patients will first be scheduled with one of our experienced Nurse Practitioners (NPs) or Physician Assistants (PAs) for a thorough initial evaluation.
Our NPs and PAs work closely with our physicians as a unified care team. Your care plan will be developed and overseen collaboratively to ensure you receive comprehensive, coordinated treatment without unnecessary delays.
This model allows us to: Minimize wait times, improve access to care & provide prompt attention to your health needs.

** Telehealth Visits **
We feel that telehealth visits can be a productive and a useful alternative to an in-office doctor’s visit, particularly for routine follow ups Telehealth visits work well to discuss patients’ symptoms and treat many Rheumatologic issues. In office visits are important to assessing physical findings such as joint swelling, rashes, etc which can not always be adequately assessed via camera.
Insurance coverage for telehealth visits is constantly changing and it is the patient's responsibility to check with their insurer as to coverage. The patient is responsible for any costs for telehealth visits not covered by insurance.
Please use a smart phone or iPad/tablet, desktop or laptop with a camera and connect to wi-fi rather than use cellular service.
PLEASE DO NOT DO THESE VISITS WHILE DRIVING.
If you are using an iPhone/Smart phone, you will receive a text with a link. If you are using a desktop or laptop you will receive an email with a link (on the date of your appointment.) that you can click on, to bring you into your doctor’s virtual waiting room for your scheduled appointment.
There are a few important things to be aware of to make your upcoming telehealth visits as productive as possible:

Please click into the link that takes you to our virtual waiting room approx. 5 minutes before your scheduled appointment time.

Be sure to complete any lab work prior to the appointment if this was requested by your physician

Be sure to know what refills you need.

Any new lab & or testing orders will be mailed to your home.

We may need to contact you prior to or right after your appointment to get more insurance information and to take copay/deductible payments in the same way we would if you were seeing the doctor in the office.
